Forms for Permits and Notifications

All permit applications relating to amusement rides are to be done through the ARiES portal.

If you are unable to access the portal, you may still do a manual application by downloading the relevant forms listed below.

Permit Purpose of Application
Installation permit Install an amusement ride
Modification permit Modify an amusement ride that already has an operating permit
Operating permit Commence operation of an amusement ride
Renewal of operating permit Renew permit to operate an amusement ride
Application for deviation To deviate from previously permitted installation or modification works


Notification Purpose of Notification
Change of qualified person Change in qualified person and the substitute qualified person
Appointment of new ride manager Appointment of a new ride manager
Minor modification Intended minor modification of the amusement ride 
Relocation Intended relocation of the amusement ride 
Relocation completion Submit the Certificate of Relocation when relocation of amusement ride is completed

Certificate of Design and Specifications Conformity QP's declaration on review of design and specification of amusement ride  To submit with application for:
- Installation Permit; and
- Modification Permit
Certificate of Inspection for Installation/ Modification Works QP's declaration on inspection of installation or modification work of amusement ride To submit with application for Operating Permit
Certification of Supervision for Installation/ Modification Works QP's declaration on supervision of installation or modification work of amusement ride To submit with application for Operating Permit
Certificate of Annual Inspection QP's declaration on inspection of amusement ride for renewal of operating permit  To submit with application for Renewal of Operating Permit
